1. Assumption of Risk
I, {name} understand and acknowledge that participation in Brazilian Jiu-Jitsu and related combat sports involves inherent risks, including but not limited to physical injury, pain, discomfort, and the possibility of serious injury or death. I voluntarily elect to participate in classes and activities at SUB-G BJJ LLC and fully assume all risks, hazards, and dangers related to such activities.

2. Release of Liability
I, {name} on behalf of myself, my heirs, executors, and assigns, hereby release, waive, and discharge SUB-G BJJ LLC, its owners, instructors, employees, volunteers, and any other participants from any and all claims, liabilities, damages, or causes of action arising out of or related to my participation in activities at SUB-G BJJ LLC. This release includes, but is not limited to, claims for personal injury, property damage, negligence, or wrongful death.

3. Medical Consent
I, {name} declare that I am physically fit to participate in Brazilian Jiu-Jitsu and other combat sports activities and that I do not have any medical condition that could impair my participation or put me at risk. In the event of an emergency, I authorize SUB-G BJJ LLC to seek medical treatment on my behalf if I am unable to do so, and I agree to assume responsibility for any medical expenses incurred.

4. Indemnification
I agree to indemnify and hold harmless SUB-G BJJ LLC, its owners, instructors, employees, volunteers, and other participants from any and all claims, damages, costs, or liabilities arising out of my actions or conduct while participating in any activities at SUB-G BJJ LLC.

5. Acknowledgement of Safety Rules
I agree to comply with all rules and safety instructions provided by SUB-G BJJ LLC, its instructors, and staff. I understand that failure to follow these rules may result in my exclusion from participation in activities at SUB-G BJJ LLC.

6. Photography/Video Release
I understand that photographs and/or videos may be taken during classes and activities for marketing, promotional, or educational purposes. I grant SUB-G BJJ LLC the right to use such media, without compensation, for promotional and advertising purposes.

7. Governing Law
This waiver and release shall be governed by the laws of the state of New York. Any disputes arising from this agreement shall be resolved in the courts of Orange County, New York.

8. Acknowledgement of Understanding
I {name} have read this waiver and release, fully understand its terms, and sign it voluntarily. I acknowledge that by signing this agreement, I am giving up substantial rights, including the right to sue SUB-G BJJ LLC.
